Pain-related Complex Cancer Late Effects Rehabilitation Service (CCLERS): Referral
Overview and Contact Information
We are a national specialist service commissioned by NHS England, accepting referrals from throughout the UK and internationally.
Referrals can be accepted from any health professional and there is no cost for referral for patients registered with a GP in England.
Our clinical services are available to adults aged 18 years and over, who meet the following criteria:
- Active cancer treatment ended at least 12 months ago.
- Experience severe and persistent pain.
- Experience reduced physical function or mobility.
- Have seen local pain/rehabilitation/late effects services without improvement in symptoms
- Local late effects services consider patient complex.
Exclusions
- Under 18s. This is an adult service.
- CCLERS does NOT offer medical intervention/invasive procedures.
- Not yet received any pain medication advice.
- Requires acute hospital care.
- Severe physical or psychological co-morbidities which would impede travel or ability to engage in physical rehab.

Referral Guidelines
Referral to the Complex Cancer Late Effects Rehabilitation Service (CCLERS) can be made as follows:
GP Referrals
The service is commissioned to accept GP referrals only if they are submitted via the e-Referral Service (e-RS).

Once a patient has been referred to the service our admin team will contact the registered GP by email to request the supporting information as outlined on this page.
Supporting information
The following information should be presented within the referral letter and notes:
- Cancer diagnosis, last active treatment, the treatment thus far, procedures to date, current therapy and the current problem and the reasons for referral.
- Copy of all clinical letters from all local services that have been involved in the patients care within the last 12 months (i.e. Pain Consultants, Rheumatologists, Cancer Specialists, Mental Health, Physiotherapy, etc.)
- Copy of all clinical discharge letters from any previous admissions
- All results of scans (MRI, CT, etc) from the last 12 months
- The most current Patient Summary
Failure to include or provide this information may result in the referral being declined.
Funding
Prior funding approval is not required for patients registered with a GP in England.
For people outside of England, local area funding approval will need to be obtained before a referral is submitted.
